Registering your community allows you to access your Smoke-Free Community Assessment Tool (SFCAT). Registration is simple and takes less than five minutes.

You can register for one community, several communities within a single county or for an entire county.

The first step is to identify the county in which the community (ies) of interest are located. Registration is done by county and then community (ies) to make sure there is only one SFCAT being completed at any given time for any given community and to encourage collaboration with other agencies in the same community or county to complete the assessment.

STEP 1: Please click on the county in which the community (ies) of interest are located (the community (ies) for which you wish to complete the Smoke-Free Community Assessment Tool or SFCAT).

Click here if the community/county that you want to complete the SFCAT for is not in Michigan or if you do not represent a community/county but would like to access and view the online SFCAT.
