If your community and/or county is outside of Michigan, if you are not representing a community or county, or if your community or county is not listed (or is already registered), please contact us to obtain special registration instructions – via email at SFCAT@mihealthtools.org.

Please do not register as a community simply to “test” the online SFCAT as that will prevent that community from registering.

We want all interested communities to have access to the Smoke-Free Community Assessment Tool and we want to encourage collaboration among interested parties within a community.
