The Information We Collect & How It Is Used

  • Identifying Information:

    When you complete the online Success Story application, we will collect the following personal identifying information:

    • The name of the person submitting the success story
    • The submitter's email address
    • The submitter's phone number
    • Your school's or school district's postal address

    In addition to using this information to credit your school/school district with its success, we use the submitter's email address and name to communicate any questions or updates related to your application and its publication. For example, we send an email to confirm that your success story was submitted successfully. We use your phone number as a back up for contacting you (we generally use email as our first method) with any questions regarding your application. We may also use your phone number to respond to any questions you may ask that are more easily answered via phone. We use school postal addresses to confirm a school's identity as many schools have similar or identical names. If you request that something related to the Healthy School Success Stories be mailed to you via the school or school district office or if there are materials that need to be mailed to you related to the program, we will also use the school's/school district's address for that purpose.

    The school/school district name will be published on the Mihealthtools online map (www.mihealthtools.org) as part of recognizing your accomplishments. The submitter's information and the school/school district information will also be posted on this website when the success story is published to properly recognize the school/school district and to provide other Michigan schools with contact information should they want to know more about the success story. This contact information will be used only for the purpose of administering the success story program and this website (including the online application form) and facilitating communication and recognition with other Michigan schools. The information will not be used or shared in any other way.

  • Information About School Environments: Via the Success Story application, we also collect information about various aspects of school environments that pertain to asthma management, physical activity, healthy eating, sun safety, violence and injury prevention, and tobacco use prevention. The purpose of collecting this information it to provide a context for the success story. Like the contact information provided, it will be published along with the success story.



    Completion of the Success Story Form is voluntary. Choosing not to complete the application will not affect the services you or your school/school district receive from the sponsoring organizations.

Opt Out Policy

Schools/school districts that begin the Success Story Form may stop entering information on the application at any time. Participation is always voluntary. All you need to do is stop recording information. If you have submitted a Success Story and would like to retract it, please contact the Healthy Schools Support Team to avoid publication and to be removed from the database and any contact lists (via email at healthy.schools@earthlink.net).
