How to Complete the Online Success Story Form

Completing the online success story is easy and should take less than 15 minutes if you have all the information you need to complete the form. You must complete the Success Story Form in one sitting. You cannot save your answers and come back later.

Here's how to complete and submit a success story:

  1. Click on the Submit a Success Story menu tab (above) or click here to begin.
  2. Follow the steps and instructions as indicated on the form, selecting the relevant county and school district.
  3. Answer the first question on the form that asks whether the success occurred at the school level or the school district level. (The questions that appear are slightly different for school level vs. school district level success stories.) Then, complete the rest of the information on the form.
  4. Be sure your success story is concise and is about a single topic or success. Do not combine several success stories into one success story submission. Remember: you can submit as many success stories as you like.
  5. When you have completed all the required information, click the submit button at the bottom of the success story application. The success story software will check all your answers to make sure they are complete and there are no inconsistencies.

    If there are problems, they will be noted in red font next to the question. Correct the errors and submit again.

    When your success story application is successfully submitted, you will receive a confirmation note on the page. You will also receive a confirmation email.

You can submit as many success stories as you like. Just come back to this website, click the Submit a Success Story menu tab and follow the steps outlined above.

What Happens After You Submit Your Success Story

Our administrative staff will review your success story for clarity and final categorization and will contact you with any questions. Please allow at least two weeks for this review - it may take more or less time depending on the number of success stories to be reviewed at any given time.

Once your success story is reviewed and approved, it will be published on this website in the Review School Success Stories section. The published stories will be organized by categories, such as Healthy Eating or Tobaccco-Free, to enable schools to easily locate the types of stories they would like to read. Although most of the success stories submitted will be published, the administrators reserve the right to withhold or delay publication of any story that is submitted.

Tips for Completing the Online Success Story Form

Because the online success story form must be completed in one session, you'll want to come prepared to answer all the questions.
  • Take a look at the preview/print version of the form available in the Preview the Success Story Application Form section of this website. You can preview and print it or download a PDF copy of a form that looks very similar to the one on this website.

  • Using the preview copy, collect or note any information you might not know off the top of your head.

  • The success story narrative or description is limited to 2000 characters. Consider writing the story narrative in advance using Word or similar word processing software that can give you a character count prior to moving it onto the online form. Creating a Word document or other electronic file may also help you in the following ways:
    • You can run spellcheck and grammar check to ensure the story you submit is typo-free. You can also ask a colleague to proof it for you ahead of time. What you write on the online form is what will be published on the website. The online form does not have spell or grammar check capabilities.
    • You can cut and paste from your Word document into the designated area of the online form, thereby avoiding tedious retyping.
These are just suggestions of ways to complete the form efficiently. There are no right or wrong approaches.
