The Michigan Healthy Communities Collaborative is committed to protecting your
privacy and to making sure you know about the privacy policies and practices
associated with the Promoting Healthy Eating website and the Nutrition Environment Assessment Tool (NEAT) online assessment.
This page discloses the privacy practices for this website and for the online assessment.

The Information We Collect & How It Is Used
- Identifying Information: When you register your community to
complete the Nutrition Environment Assessment Tool online assessment,
we collect the following personal identifying information:
- Your primary contact’s name
- Your primary contact’s email address
- Your primary contact’s phone number
- Your primary contact's postal address
- Your primary contact's job title/position
In addition to using this information to register your community for the online
Nutrition Environment Assessment Tool, we use your email address and name to
communicate any updates or reminders related to your assessment and its completion.
For example, we send an email to confirm your registration and to confirm that we have
received your completed assessment data. We use your phone number as a back up for contacting
you (we generally use email as our first method) with any important information
regarding your assessment. We may also use your phone number to respond to any
questions you may ask that are more easily answered via phone. We use your
title/job position and postal address if you request that something related to the
assessment be mailed to you.
Your personal identifying information will never be shared with anyone except the
NEAT administrative team (staff and consultants) unless we contact you and obtain your permission.
It will be used only for the purpose of administering this website (including the
online assessment forms) and effectively communicating with you (and those who are
working with you to complete the NEAT) about issues related to your assessment and
about news related to this website.
- Information about Your Community and Community Environment: We also
collect your community’s name and via your answers to the NEAT, information about
various aspects of your community’s environment that pertain to healthy eating.
In general, the responses you provide on your assessment will be summarized,
along with responses from other communities, with no identification of your name or
other identifiers which would connect responses with individuals or specific
communities. Individual community level responses may occasionally be reported but
will not be associated with any information that would identify the community or the
individuals who completed the form, unless you provide permission.
Please note that the Michigan Healthy Communities Collaborative will be publicly recognizing all communities that complete the NEAT by placing them on a map of Michigan Health Tools Assessment Participants (at www.mihealthtools.org). We will publicize only the fact that communities have participated in NEAT, not communities' scores nor communities' responses. We may also publicize NEAT participation through other channels, such as press releases, presentations, and on other Michigan Health Tools websites.
Completion of the Nutrition Environment Assessment Tool is voluntary.
Choosing not to complete the assessment will not affect the services you or your
community receive from the Michigan Healthy Communities Collaborative or the partner organizations that comprise it.

How Your Information Is Protected
This site has security measures in place to protect the loss, misuse, and
alteration of the information under our control. Your name, email address,
phone number and school data are maintained on secured files accessible only by
the NEAT administrative team (staff and consultants). However, as with any Internet endeavor,
absolute security of information transmitted through channels (communication
networks, hardware, software) not directly operated or controlled by us or our
partners cannot be guaranteed.

How to Update Your Information
We rely on you to provide updates to information that you have provided to us.
Registered communities can update email address and primary contact name,job title/position,
postal address and phone number by logging in with the old email address. After
logging in, click on the Update Your Info menu item and follow the instructions
provided. Changes to the responses you provide on the Assessment can be changed any
time prior to submitting it as complete, simply by logging in and changing your
answers. For other information that you would like updated, please contact us.

Opt Out Policy
Communities that register may stop using their assessment at any time.
Participation is always voluntary. All they need to do is stop logging in and
recording information. If you no longer wish to receive emails about the
Nutrition Environment Assessment Tool and this website, please contact
Stephanie Halfmann (shalfma@mphi.org ) to request removal from the email list.

Use of Cookies
We use cookies on this website for those who register to complete the online
Nutrition Environment Assessment Tool to allow us to personalize your experience
when you log in—that is, to display the information you have already entered for
your community, to allow you to record further information and to provide feedback
(scores based on the data you supply). No personal identifying information is
stored on any cookie. If you do not accept cookies, you will not be able to log in
and record your community’s information.

Notification of Changes to This Policy
In the unlikely event any substantial changes are made to this privacy policy,
community registrants will be notified via email (provided you are still on
our email list). Examples of substantial changes include changes in the way your
identifying information or community data will be used or shared.
Minor changes also may be made (for example to clarify points based on user
feedback or to change contact information if there are changes in administrative
staff). Those who have signed up to receive information or registered to complete
the NEAT will not receive email notification of such minor changes; the most recent
version of the policy will always appear on this website.
