The sponsoring partner organizations or the Healthy School Environment Recognition Program are committed to protecting your privacy and to making sure you know about the privacy policies and practices associated with the Healthy School Environment Recognition Program, including this website and the online application. This page discloses the privacy practices for the Recognition Program, this website and the online Recognition Program application.

The Information We Collect & How It Is Used

  • Identifying Information:

    When you register to complete the online Recognition Program application, we will collect the following personal identifying information:

    • Your primary contact’s name
    • Your primary contact’s email address
    • Your primary contact’s phone number
    • Your school’s postal address

    In addition to using this information to register your school for the online Recognition Program application, we use your email address and name to communicate any updates or reminders related to your application and its completion. For example, we send an email to confirm your registration and to confirm that we have received your application after you submit it. We use your phone number as a back up for contacting you (we generally use email as our first method) with any important information regarding your application. We may also use your phone number to respond to any questions you may ask that are more easily answered via phone. We use your school’s postal address to confirm your school’s identity as many schools have similar or identical names. If you request that something related to the Recognition Program be mailed to you via the school or if there are materials that need to be mailed to you related to the program, we will also use the school’s address for that purpose.

    Your personal identifying information will never be shared with anyone except the Healthy Schools Support Team and the partner sponsoring organizations unless we contact you and obtain your permission. It will be used only for the purpose of administering the program and this website (including the online application form) and effectively communicating with you and your team about issues related to your application and about news related to the Recognition Program.

  • Information About Your School's Environment: Via the Recognition Program application, we also collect information about various aspects of your school environment that pertain to physical activity, healthy eating, sun safety, and tobacco use prevention. The purpose of completing the application is to review the accomplishments your school has made toward creating a healthy environment. Although the accomplishments your school reports in the Recognition Program application take time to achieve, it will only take about 30 minutes for your school to answer the application questions online.

    The responses you provide on your application will be summarized and schools that achieve a recognition level will be publicly recognized and applauded. Schools that attain a recognition level will be published on this website and celebrated. Schools may also be recognized in press releases and other media. We will publish your school's information if you submit an application and achieve a recognition level as part of the Recognition Program process. We may also publish your school's information in conjunction with the success story that you submit as part of the Recognition Program application.

    Completion of the Recognition Program application is voluntary. Choosing not to complete the application will not affect the services you or your school receive from the sponsoring organizations

How Your Information Is Protected

This site has security measures in place to protect the loss, misuse, and alteration of the information under our control. Your name, email address, phone number and school data are maintained on secured files accessible only by the Healthy Schools Support Team (the website administrators and the partner organizations). However, as with any Internet endeavor, absolute security of information transmitted through channels (communication networks, hardware, software) not directly operated or controlled by us or our partners cannot be guaranteed.

How to Update Your Information

We rely on you to provide updates to information that you have given to us. Registered schools can update email address and primary contact name, postal address and phone number by logging in with the old email address. After logging in, click on the Update Your Info menu item and follow the instructions provided. Changes to the responses you provide on the application can be changed any time prior to submitting it as complete simply by logging in and changing your answers. For other information that you would like to update, please contact us.

Opt Out Policy

Schools that register may stop entering information on their application at any time. Participation is always voluntary. All they need to do is stop logging in and recording information. If you no longer wish to receive emails about the Recognition Program and this website, please contact us to be removed from the contact list (via email at healthy.schools@earthlink.net.

Use of Cookies

We use cookies on this website for those who register to complete the online Recognition Program application to allow us to personalize your experience when you log in—that is, to display the information you have already entered for your school, to allow you to record further information and to provide feedback (scores based on the data you supply). No personal identifying information is stored on any cookie. If you do not accept cookies, you will not be able to log in and record your school’s information.

Notification of Changes to This Policy

In the unlikely event any substantial changes are made to this privacy policy, you will be notified via email (provided you are still on our email list). Examples of substantial changes include changes in the way your identifying information or school data will be used or shared.

Minor changes also may be made (for example to clarify points based on user feedback or to change contact information if there are changes in administrative staff). Those who have signed up to receive information or registered to complete the Recognition Program application will not receive email notification of such minor changes; the most recent version of the policy will always appear on this website.
