Welcome! The Michigan Healthy Communities Collaborative,
comprised of partner organizations throughout Michigan, has developed online assessments and
websites to help promote and support better health for Michigan citizens – in communities,
at work, at school and in places of worship. Below are brief descriptions of the assessments,
as well as a map showing the communities and organizations that have participated.
Healthy Community Checklist : Communities can get a snapshot of their health environment by completing this online checklist.
Healthy Schools, Healthy Students : The Healthy Schools websites feature the Healthy School Action Tools (HSAT) , Healthy School Environment Recognition Program , Healthy Schools Success Story website, and Student Health Survey website.
Promoting Active Communities : Communities can see how well they support and promote physical activity and become eligible for the Michigan Fitness Foundation Active Community Award .
Promoting Healthy Eating : Communities can use the online Nutrition Environment Assessment Tool (NEAT) as the first step in improving their support of healthy eating.
Creating Smoke-Free Communities : Communities can use the Smoke-Free Communities Assessment Tool (SFCAT) to help increase smoke-free environments in ALL community venues.
Healthy Work Environments : Employers can use the Designing Healthy Environments at Work (DHEW) assessment to identify ways to create a healthier work environment.
Walk by Faith : Two websites providing resources to assist Michigan faith leaders in promoting healthy congregants: one features the Promoting Healthy Congregations assessment and the other is the online home of the Michigan Faith-Based Health Association .

Community Coalition Assessment Tool :
Community coalitions can use the assessment survey and feedback reports to identify strengths & improvement needs in 6 important areas of function.
